About OBGYN West Clinics
OBGYN West is a physician-owned, multispecialty practice that is committed to delivering high-quality women’s health care, every day.
Our team includes board-certified OB & GYN physicians, a board-certified urogynecologist, a family medicine physician, physician assistants, and a nurse practitioner.
Women’s Care Every Step of the Way
Since 1970, we’ve specialized in women’s health, whatever the stage of life. “Every step of the way” was coined years ago and still holds true today. Our services include fertility, pregnancy and after-baby care, breast-feeding support, well-woman care, weight management services, and routine and complex care for gynecology concerns. Our mammography center, the Breast Center of Minnetonka, offers screening mammograms.
We specialize in minimally-invasive surgery with and without robotics, bladder concerns, and pelvic organ prolapse. Our team includes an OBGYN physical therapist and a registered dietician who helps moms-to-be and new moms navigate gestational diabetes.

OBGYN West clinics are conveniently located in Minnetonka, Chaska, Eden Prairie, and Waconia. We deliver babies at M Health Fairview Southdale Hospital and Ridgeview Medical Center, and perform surgery at the same as well as at area surgery centers.
Our History
OBGYN West was founded by four OBGYN physicians who came together to deliver a higher-quality of women’s health care. From two clinics in Minnetonka and Minneapolis, we’ve expanded our services to better serve generations of women who live in the west and southwest suburbs of the Twin Cities.

FMLA-Minnesota
Effective January 2026, the Minnesota Paid Family and Medical Leave Act helps provide partial wage replacement for medical leaves, postpartum recovery, and bonding time. You must apply and qualify for Paid Leave.
Our clinic can help with medical documentation once you’ve completed your portion of the documentation and submitted it to our office. You will need to confirm with HR if your employer uses the state program or a private vendor.
Minnesota Paid Leave: Pregnancy + Postpartum + Bonding
What You Need to Know
- Minnesota Paid Family and Medical Leave Act helps to replace part of your paycheck when you need time off for medical needs, recovery after birth, and bonding with your baby/adoptee/foster.
- You must apply and qualify for a Paid Leave. You apply through the state, not our clinic. To learn more and access the leave request form(s) you need, visit https://pl.mn.gov/toolkits/individuals-and-families-toolkit
- When you apply for Paid Leave, you will need to provide certification document(s) that verify your reason(s) for taking a Paid Leave. If you are requesting a Bonding Leave, the documentation may come from the hospital or our clinic.
- Apply no earlier than 60 days before your leave begins.
- Complete the form(s) that matches the type of leave you are requesting. We will assist with certification for the medical portion of the form.
- If you qualify, you can take up to 12 weeks of Medical Leave (for pregnancy/childbirth
recovery) and up to 12 weeks of Family Leave (for bonding), with a combined maximum
of 20 weeks in a benefit year. - Bonding Leave must be used within 12 months of birth (or adoption/foster placement).
- You will need to submit either a hospital discharge summary or a birth certificate for verification.
Our Clinic Helps You Document Your Paid Leave Request
- Complete the Leave form that matches your request. To learn more or access forms, visit https://pl.mn.gov/toolkits/individuals-and-families-toolkit
- If you are requesting a Bonding Leave, the hospital can help provide this documentation.
- Once you’ve finalized your portion of the form, submit it to our clinic in one of three
ways:
o Bring your printed form to your provider or the front desk at your clinic
o Submit your paperwork in the portal after you have completed your portion of the form
o Email it to your clinic: OBGYN West: medicalrecords@obgynwest.net - Once our team receives your form AND the documentation is final, we will review and process it within 15 business days.
- You will need to view processed documentation on a computer or laptop due to encryption; do not try to view on your mobile phone.
- Our team will certify six weeks for vaginal delivery and eight weeks for C-section. Requests for Bonding Leaves cannot be requested or approved until after the baby is born. Apply no earlier than 60 days before your leave begins.
- For eligibility, pay details, and job protection at your workplace, your HR team and the state program are the best resources.
